In the process of depathologizing barebacking, I argue nonetheless that the subculture cannot be understood in purely rationalist terms—that, in other words, one still needs an account of the unconscious to explain what’s at stake. Previous attempts to depathologize barebacking have framed the issue in terms of rational risk assessment, claiming that protease inhibitors usher in “altered realities” that alter gay men’s sexual decision making. In my view, such arguments too readily overlook those nonrational motives that lead men to act against what may be regarded as their own best interests.
